Restoring Hope In Autism:
MyGOAL Inc. 2012 Enrichment Grants
MyGOAL Inc. is proud to offer a grant program that will
enable families to take advantage of socialization and educational
opportunities designed for individuals with special needs. Such interactive
activities are beneficial for the development of individual(s) with Autism
Spectrum Disorders, but often times these programs are not covered privately or
by other third-party funding sources such as school districts. The purpose of
the grants is to enrich the body, mind, and spirit of individual(s) with Autism
Spectrum Disorders, resulting in a higher quality of life. Although awarded to
the primary care-giver, it is with the understanding that the grant will be
used to benefit the individual(s) with Autism Spectrum Disorders.
MyGOAL Inc. is now accepting applications for the MyGOAL
2012 Enrichment Grant program. Applicants who satisfy the grant criteria,
complete the application, and meet the deadline (Postmarked no later than
March 15th) will be considered for a grant. MyGOAL Inc. Enrichment
Grants will be based upon a different theme each year, and will be
awarded based upon a combination of perceived benefit to the individual(s) with
Autism Spectrum Disorders and family economic need.
This year eight (8) $250 grants will be awarded based upon the theme “recreation and physical activity” and would include opportunities
such as:
1. Camp
2. Sports
3. Exercise
4. Dance
5. Physical Therapy
6. Swimming
Enrichment
Grant guidelines:
The following must be received by MyGOAL in
order to be considered for an Enrichment Grant
Applicants must be the
primary care-giver of the individual on the Autism Spectrum and provide the
following:

Verification of Diagnosis for ASD

Indication of Household Income (subject to
verification if requested by the Award Committee)
-
Number of dependents with ASD (a separate
grant application may be submitted for each dependent)

Name and overview information for the recreation/physical
activity, contact information for the organization offering the
program/activity, duration and total cost of the program/activity
-
Short explanation of how the
recreation/physical activity will benefit the individual(s) with ASD
-
Confirmation that the recreation/physical activity
will begin this calendar year

Completed, signed and dated Enrichment Grant
Application
Please note: Individuals are still eligible to apply for
the annual Family Grant available later in the year. If the family is awarded
an Enrichment Grant but later decides the individual(s) with ASD will not
participate in the activity for which the award was granted, then they should
immediately notify MyGOAL Inc. so that the matter can be assessed

MyGOAL established the 2009 Daryl Matthew Gosein Scholarship Fund in loving memory of Daryl Gosein. Individuals who hear about Daryl’s story tend to say “Oh, what a tragedy.” However, those who knew Daryl know what an inspiring and rewarding experience it was to be a part of his short life. Daryl fought a tough, daily battle against Autism; a battle he fought with great love and a smile on his face. On his path to recovery, Daryl overcame AMAZING obstacles; some that baffled the medical community.
The Daryl Matthew Gosein scholarship will provide funding that will enable families to pursue treatments that Daryl never got a chance to complete. Additionally, it will also provide financial assistance to future teachers in the Autism community, and honor current special education teachers or therapists. In spite of their own personal loss, it is an honor for Daryl’s family to support the families of individuals on the Autism Spectrum as they continue on their journey towards recovery or a better quality of life.
*Research shows that autism is treatable by providing early intervention through various types of therapies. The sooner a child receives therapy, the more likely the child will learn how to interact with others socially and integrate in mainstream schools**

The Daryl Matthew Gosein Memorial Scholarship funds will be disbursed for the following programs:

- How has your nominee demonstrated dedication to individuals (and/or their family members) on the Autism Spectrum?
- To the best of your knowledge, in what Autism-related events/ programs have your nominee participated? Please identify any leadership roles in those events/programs?
- In your opinion, what would this acknowledgment mean to your nominee?
- For how long, and in what capacity, have you known the nominee?
- Is there another role or position to which you would recommend your nominee? (If yes, please identify).
- Medical Needs
To assist in paying non-reimbursable medical expenses, including first-time visit to a Defeat Autism Now (DAN) Doctor or other Autism-related specialist
- Nutritional Needs
To purchase vitamins or other nutritional supplements that are specifically designed for children with ASD.
- Personal Needs
To provide personal needs for the individual with ASD.
- Proof of household Income
- Number of dependents
- Number of dependents with Autism Spectrum Disorder
- Information about what current funding the grantee is receiving (i.e. medical)
- Completed, signed and dated Grant Application
- Verification of diagnosis
- No more than 250 word description of current family situation
- (grant application over this word amount will not be considered)
- Copy of previous year’s tax return

MyGOAL “Family Grant Award” Program
The 2011 MyGOAL Family Grant Award Program is a need-based grant for treatments (including vitamins and other nutritional needs) that may not otherwise be covered privately or by other third-party funding sources such as school districts, county programs, insurance, and/or other grant making entities. Although awarded to the primary care-giver, it is with the understanding that the grant will be used to benefit the individual(s) with Autism Spectrum Disorders.
> Two (2) $1,000 Family Grants will be awarded for the following purpose(s):

MyGOAL “Outstanding Autism Service Award” Program
The 2011 MyGOAL Outstanding Autism Service Award Program is established to acknowledge outstanding special education teachers, paraprofessionals or therapists (speech, occupational or physical therapy) in the instruction of individuals with Autism Spectrum Disorder. The award is given to the individual who has demonstrated commitment and dedication to the education or therapy of individuals on the Autism spectrum. The Awardee will be selected following a nomination by another individual.

Grants & Service Awards Will Be Given Out Once A Year.
The initial award will be given to families in the tri-state area (New York, New Jersey, Pennsylvania) on or before November 5, 2011 or at the Gala event. The scholarship committee will review all applications and select awardees.

Application for awards will be made available on June 1, 2011 and due no later than July 31, 2011 (postmarked).
The Scholarship committee will review all applications by September 7th and notification to scholarship recipient(s) will be mailed by September 20, 2011.
> Ineligibility (applies to both):
Members of MyGOAL Inc.’s Board of Directors and Advisory Board and their family members are ineligible for the any of the described Grants and/or Awards.
